1. Login to Evaluation Kit.
2. From the navigation select Report Builder.
3. Click Begin.
4. Name the report.
5. Check the boxes for all of the Projects to be included in the report, one or multiple. Then, click Add button.
6. Click Continue button.
7. Select desired survey questions.
8. Under Instructor Criteria, select Last Name or email address, then equals, then type their name (or email). Click Save.
9. Click Continue button.
10. Select Template #7, then click Generate button.
